I am the parent or legal guardian of the participant registered in programs operated by Vangalaxy Soccer Academy. I understand that participation in soccer training, camps, games, fitness activities, and related events involves inherent risks, including but not limited to physical injury, illness, concussion, sprains, fractures, dehydration, collisions, falls, emotional stress, property damage, and other unforeseen risks that may arise during participation or travel to and from activities.
I confirm that the participant is physically and mentally fit to participate in Academy activities and that I have disclosed any relevant medical conditions, allergies, injuries, or limitations that may affect participation.
I understand and acknowledge that, despite reasonable supervision, instruction, and safety measures, accidents and injuries may still occur. I voluntarily accept and assume all risks associated with participation in Academy activities on behalf of the participant.
To the fullest extent permitted by the laws of the Province of British Columbia and the laws of Canada applicable therein, I hereby release and discharge Vangalaxy Soccer Academy, its owners, coaches, trainers, staff, volunteers, contractors, affiliates, and facility providers (collectively referred to as the “Released Parties”) from any and all claims, demands, actions, liabilities, damages, costs, or expenses arising from or related to participation in Academy activities, except where caused by gross negligence or willful misconduct.
I also agree to indemnify and hold harmless the Released Parties from any claims brought by or on behalf of the participant arising from participation in Academy programs.
In the event of a medical emergency, I authorize the Academy to obtain emergency medical treatment for the participant if I cannot be reached immediately. I understand that all related medical expenses are my responsibility.
I understand that photos or videos taken during Academy activities may be used for promotional, educational, or social media purposes unless I provide written notice requesting otherwise.
By selecting “I Agree” and submitting this registration form, I confirm that: I have read and understood this Waiver and Release of Liability; I understand that I am giving up certain legal rights on behalf of myself and the participant; I voluntarily agree to these terms.
